This is the Health Office updating you on a service that will be offered at BK.
This year Wright County Public Health will be teaming up with BK to offer flu shots to all students in grades PS-12. Flu shots will be offered to all students on Tuesday, October 26th, 2021. Cost for the flu vaccination is billable through your child's health insurance (usually covered 100%) or available for $26.00 cash or check. Please see the attachment link for further details.
Highlights:
--Please print the flu shot consent form and complete it. If I do not receive this completed consent form, your child will NOT receive the flu shot.
--If you would like me to send a hard copy of the flu shot consent form home, please contact either office and I will send it home with your child.
--Please send a copy of your child's insurance card or cash/check with consent form.
--Please return the completed consent form Friday, October 22nd.
