This is the Health Office updating you on a service that will be offered at BK.
Once again Wright County Community Health will be teaming up with BK to offer flu shots this year to all students in grades PS-12. Flu shots will be offered to all students on Friday, October 21, 2022. Please click on THIS LINK to download the form and for further details.
Highlights:
--Please print the flu shot consent form. If we do not receive this completed consent form, your child will NOT receive the flu shot. You also must include a copy of your insurance card with consent form. Please note on the form if your child does not have insurance.
--If you would like me to send a hard copy of the flu shot consent form, respond to this email and we will send it home with your child.
--Please return the completed consent form Wednesday, October 19th.
--NO late consent forms will be accepted--
